Early one April morning in 1989, during a routine training exercise in the Caribbean, the center gun of Turret Two on the battleship USS Iowa blew up. There were 47 fatalities as the fireball released clouds of gases and ignited bags of powder which set off further explosions. The botched investigation began mere hours after the disaster. The Captain, an Annapolis football star who had just recently taken command of the Iowa, declined assistance from a professional accident team aboard a nearby aircraft carrier. He ordered clean up that saw immense steel plates and equipment tossed overboard. The continued investigation on land faired no better. Key evidence was lost. Testimony was twisted. There surfaced an effort to place blame on a seaman who had died in the explosion and that blame ultimately destroyed careers up the chain of command in the Navy.
